4. Seller's Permit Account Number
5. (Insurance number) if you have employees

If you do structural work, such as installing roofs, or framing, you must obtain a state contractor's license before applying for a single trade, such as handyman's permit.

After you obtain your state general contractor’s license, submit it along with the handyman's license application, a copy of your photo ID, passing ICC scores for Class A, B, and C contractors, general liability and workers’ compensation insurance, and all applicable licensing fees.
